At the STAR Health Centers, our primary objective is to provide quality health services to all members of the community. We provide comprehensive healthcare and support services to adults 18 and above, including primary and preventive care, specialized HIV care, mental health and substance use treatment, care coordination, and support programs specifically for families and older adults living with HIV and people with chronic conditions who are returning home from incarceration.

We have received quality of care awards from New York State Department of Health and the U.S. Health Resources and Services Administration. In addition, STAR Health Center-Downstate is proud to be recognized as a Level 3 Patient-Centered Medical Home (PCMH) by the National Committee for Quality Assurance (NCQA), the highest designation for quality in ambulatory care.

Because we understand the critical role access to healthcare plays in community wellness, we have established two primary locations in Brooklyn. Our first site – STAR-Downstate – was established in 1992 at University Hospital at Downstate on the campus of SUNY Downstate Health Sciences University. Our second site – STAR Brookdale – was established in 2017 at Brookdale Hospital Medical Center, which is part of the One Brooklyn Health system.
